1. Your ad size choice - ads come in 2 sizes:

• Tribute Ad - 1 block = $40       (2.25” tall x 3.5” wide)
• Tribute Ad - 2 blocks =  $70    (4.75” tall x 3.5” wide)

2. What you want printed in the ad (the words)

3. Any pictures or images (original jpg or gif files)

4. Your agreement to add the payment to your student's band account - give the full name of your band student. (This payment option is only available for OHS band parents.)
